Job description

Improving Health Equity One Message at a Time! Many of us have had the experience of doing good work, but wondering if it is truly moving the needle. At CareMessage, our mission is clear: improve health equity for low-income populations across the United States. Every role here connects directly to that purpose. CareMessage is the technology non-profit building the largest patient engagement platform for low-income populations in the United States. Powered by the Health Equity Engine™, the platform enables organizations to combine messaging, data, and interoperability to increase access to care, improve clinical outcomes, and address social drivers of health. With 22 million patients reached since 2013, CareMessage is the only patient engagement solution proven to improve health equity at scale. The team, many with lived experiences in these communities, leverages a nonprofit model to reinvest revenue into impact. CareMessage is the partner of choice for organizations committed to advancing health equity. We are a team of people who hold both compassion and accountability. We care deeply about the communities we serve, and we also hold a high standard for performance, collaboration, and integrity. Our ideal team members: - Deliver meaningful, measurable results - Invest in their teammates and build trust - Seek feedback and continually grow - Stay steady and solutions-oriented in ambiguity - Lead with integrity and responsibility - Communicate with clarity and empathy If you are motivated by impact, energized by collaboration, and ready to do some of the most meaningful work of your career, we encourage you to apply. If this sounds like you, we would love to meet you. _____________________________________ ROLE OVERVIEW The Engineering Manager, Data & Clinical Integration owns and drives the technical direction of the two teams responsible for CareMessage's data — the Clinical Integration team, which connects CareMessage to electronic health records (EHRs) and other clinical data sources (e.g. HIEs, Payor Data, etc.) across our customer base, and the Data team, which turns that data into trustworthy insights. CareMessage supports over 400 safety-net healthcare organizations including federally qualified health centers and free clinics that care for low-income and underserved patients nationwide, and these two teams are the engine behind our ability to measurably demonstrate impact on clinical outcomes, access to care, and the social drivers of health. This is a leadership role over existing teams: their systems and roadmaps are already in place, and your job is to lead, deepen, and scale them. A core part of this role is leading both teams to turn clinical data from our customers' many different EHRs into trustworthy, query-ready data that can measurably demonstrate health outcomes — and scaling those insights through both AI/ML and human analysis. The Clinical Integration team builds and maintains the integrations that connect CareMessage to leading EHRs (Epic, eClinicalWorks, NextGen, AthenaHealth, and others) over HL7, FHIR, Proprietary APIs, and direct-database interfaces using the Rhapsody integration engine, and it owns the first step of the clinical data pipeline — converting clinical data to FHIR and landing it in our FHIR Store and BigQuery. The Data team builds on top of that with a medallion (bronze/silver/gold) architecture in BigQuery using native Google Cloud services, and dbt, surfacing insights through Metabase dashboards such as our Health Equity and diabetes-outcomes views; and increasingly powering AI/ML-driven workflows that surface insights at scale. In this role, you will collaborate closely with Product, Customer Success, Data, Compliance, and Engineering to design and deliver the platform and integrations that deepen patient engagement and health outcomes, with direct ownership of technical decisions and customer-facing system behavior. Our ideal candidate is an experienced Engineering Manager (technical, people and process leader) who thrives in dynamic, fully-distributed environments and is energized by the intersection of healthcare interoperability and data. They are passionate about partnering with business stakeholders to develop the platform and processes that uncover data insights, and they are dedicated to the professional growth of engineers. They view themselves as a catalyst for organizational outcomes and accomplish that through enabling high-output team productivity and continuously seek ways to elevate Engineering and the organization as a whole. Above all, they share our mission-driven commitment and take pride in contributing to our objective of advancing health equity for low-income populations. One of the strongest skills we value here is taking a scientific approach to everything we do. We rely on evidence alongside intuition, and we look for data that challenges our assumptions before drawing conclusions. Our PRDs, documentation, and wikis reflect this mindset — we use data to guide decisions and validate our work. CORE REQUIREMENTS - Proven Track Record: At least 5 years of successful experience as an Engineering Manager over Integration and/or Data Engineering teams, in two or more early-to-growth-stage remote startup environments. - Strategic Leadership: Demonstrated ability to lead and drive the delivery of long-term technical projects across multiple teams driving measurable outcomes. - Technical Proficiency: Strong technical acumen with a deep understanding of SQL, data concepts, pipelines (ELT), and cloud data warehousing. - Modern & AI-Assisted Engineering: A track record of adopting modern development practices and leveraging AI-assisted tooling (e.g., Agentic coding, harnesses, and LLM-based workflows) to raise engineering velocity and quality — with the judgment to apply them responsibly in a healthcare/PHI context. You actively coach your teams to fold these tools into their day-to-day work. - Hands-on Experience: The ability to get hands-on with SQL and data tooling when needed. (This is predominantly a management position; however, you should be able to dig in to help answer key data questions and unblock the team.) - Collaborative Expertise: Exceptional collaboration and influencing skills, working effectively cross-departmentally to craft optimal solutions while navigating multiple constraints. - Situational Awareness: The ability to discern when to dive into technical details and when to maintain a higher-level perspective when working with Engineers, Product, and Design. - Cultural Advocate: A commitment to cultivating both individual growth and a high-performance engineering culture that attracts top talent. - Mentorship: A growth-oriented mindset with the capability to guide others on their development journey. - Communication Skills: Outstanding written and verbal communication skills, especially in a fully remote environment. - Autonomous Prioritization: Proficiency in identifying and managing priorities independently across two teams, ensuring an efficient workflow. - Diversity and Inclusion: Dedication to promoting diversity and inclusion within teams, especially given our global team composition. - This role requires the candidate to reside within the United States of America. PREFERRED EXPERIENCE - Experience managing or working closely with healthcare integration / interoperability teams (HL7, FHIR) in addition to Data teams. - Experience working in a Healthcare technology environment and the requisite Security and Compliance requirements (HIPAA/PHI). - Experience introducing or scaling agentic development workflows on a team, with attention to code quality, security, and compliance. - Experience applying AI/ML to large datasets to scale insight generation (e.g., summarization, classification, anomaly detection). - Experience with our Tech Stack: - Data: GCP / BigQuery, dbt, Python, Metabase, and a medallion (bronze/silver/gold) architecture. - Clinical Integration: Rhapsody integration engine (JavaScript), HL7, FHIR, FHIR Store, mainstream EHRs (Epic, eClinicalWorks, NextGen, AthenaHealth), Ruby on Rails, Postgres, GCP. - Background in TDD engineering environments. - Experience leveraging healthcare data and foundational data science concepts. JOB RESPONSIBILITIES - Technical Leadership: You are excited about and comfortable with taking responsibility for making sure that you understand and can defend the investments and decisions your team is making, helping to challenge, redirect and support their decisions cross-functionally. Demonstrating a personal understanding of our organizational objectives, and how the specific work you are directing is the most valuable investment to help achieve those within your scope of ownership. - Directly oversee and guide the Data and Clinical Integration teams, providing consistent, constructive feedback and conducting performance reviews to foster professional growth. - You know what the difference between good and great performance looks like and you know how to get that from your team members, including when to challenge, redirect, manage up and or out. - You know when and how to advocate for new team members or missing skillsets that would help the department accomplish more. - Actively participate in the recruitment, interviewing, and retention of top-tier engineering talent, and collaborate with other people managers to nurture team engagement and company culture. - Champion the adoption of modern, AI-assisted engineering practices across both teams by evaluating new tools, setting responsible-use guardrails (especially around PHI), and continuously raising the bar on productivity and quality. - Working with engineering and product leadership, refine and execute the technical strategy for converting EHR clinical data into trustworthy, query-ready data that measurably demonstrates health outcomes. - Manage shared roadmaps and dependencies between the two teams, given that Integrations owns the first step of the clinical data pipeline that Data builds upon. - Partner with Product to scale insight generation from our clinical data — combining AI/ML approaches with human analysis — while keeping it trustworthy and compliant. - Remain close to the solutions and, when necessary, play a hands-on role helping to communicate via prototypes, or vibe-coded example applications, answer key questions via SQL or other technical tools. - Stay abreast of emerging technologies and industry trends and advocate for best practices in the department; review and collaborate with engineers on technical specifications to ensure excellence in execution. - Work closely with Product and Design to conceptualize and define impactful solutions, often through Product Requirements Documents (PRDs). - Skillfully lead and facilitate team rituals such as Kickoffs, Retrospectives, and Tech Debt Reviews, and collaborate cross-functionally to understand estimates and prioritize the backlog, making pragmatic trade-offs. - Develop processes, tools, and systems that enhance the departments' overall effectiveness. - Contribute to the team by participating in the On-Call (PagerDuty) rotation. WITHIN 1 MONTH YOU'LL: - Gain a foundational understanding of our product, customers, and patients. - Meet key internal stakeholders and begin to understand policies and protocols (including our Security and Compliance posture). - Establish rapport with the engineers on both teams through Wiki contributions, 1:1s, and team rituals as well as with key peer collaborators (e.g. Product) - Develop a working understanding of the clinical data pipeline — from EHR integration through the medallion architecture and reporting. WITHIN 3 MONTHS YOU'LL: - Complete a product demo, demonstrating your comprehension of our overall product platform. - Gain a strong understanding of our technical environment and identify areas for growth in our processes, systems, and/or tooling. - Take over responsibility for performance management and feedback for the engineers you oversee. - Have a clear, shared roadmap across the Data and Clinical Integration teams, with healthy dependency management between them. - Have implemented at least one improvement that impacts the way all of Engineering operates. WITHIN 6 MONTHS YOU'LL: - Display strong leadership across both teams and within engineering management as a whole. - Have a deep understanding of our data platform and clinical integration landscape, and refine the technical roadmap and sequencing to meet our long-term data goals while ensuring compliance with all necessary requirements. - Work with engineering leadership to recruit and hire best-in-class engineers as needed. - Make contributions to processes, tooling, or technology that impact the way the company operates. If you are US-based, a note about visas: At this time, we are not able to sponsor work visas. Applicants must be authorized to work in the United States on a full-time basis without sponsorship.
