VIRTUAL PATIENT CARE COORDINATOR - HORMONE OPTIMIZATION & WELLNESS MEDICINE

About the Role We're seeking a compassionate, organized Patient Liaison to join a growing Hormone Optimization and Wellness practice — one built around helping patients feel like themselves again, whether that means restoring energy, balancing hormones, or reaching a healthier weight in a way that actually lasts. This isn't a traditional receptionist role. It's a relationship role. You'll be the steady, encouraging voice patients hear throughout their treatment journey — checking in, answering questions, and helping them stay on track toward the results they came for. What You'll Do Patient Follow-Up & Education (Primary Focus) • Proactively reach out to patients throughout their treatment journey to check progress and encourage compliance • Discuss treatment plans clearly and confidently using approved scripts • Answer common patient questions using Top 10 FAQs • Support patients in staying engaged and consistent with their wellness goals Appointment Coordination & Outbound Calls • Make outbound calls to schedule, confirm, and follow up on appointments • Coordinate patient recall and re-engagement outreach as needed • Keep patient records and communications organized in Booker and Weave Administrative Support • Maintain accurate documentation of patient interactions and follow-ups • Support the broader front-office team with day-to-day administrative tasks as needed Required Qualifications • 2+ years of experience as a Medical Receptionist, Patient Coordinator/Liaison, Medical Concierge, or Customer Success representative • Excellent English communication skills and a warm, confident phone presence • Experience making outbound patient calls and managing follow-up • Comfortable discussing treatment plans using approved scripts • Strong organizational and multitasking skills; able to work independently in a remote setting Preferred Qualifications • Background in Hormone Optimization, Functional Medicine, MedSpa, Concierge, or Integrative Medicine • Experience with MedSpa EMR platforms or Weave • Sales, retention, or relationship-management experience • Experience with patient recall campaigns or working toward compliance goals Why This Role You'll be a genuine part of patients' wellness journeys — not just scheduling appointments, but helping people stay motivated and supported as they work toward feeling better, month after month.
